14 Intermountain Health Hospitals Receive Leapfrog “A” Grade for Patient Safety Excellence

Nine Intermountain Health hospitals also achieved a Straight “A” designation, earning an A grade for five consecutive Leapfrog reporting cycles, reflecting sustained excellence in patient safety over time.

Intermountain Health has again been recognized as one of the nation’s safest health systems for patients, according to The Leapfrog Group, a national healthcare safety and quality measurement organization that released its Spring 2026 Leapfrog Hospital Safety Grades.

Several hospitals also achieved a Straight “A” designation, earning an A grade for five consecutive Leapfrog reporting cycles, reflecting sustained excellence in patient safety over time.

Hospitals earning an “A” for Spring 2026 include:

  • Intermountain Health American Fork Hospital (Utah)
  • Intermountain Health Cedar City Hospital* (Utah)
  • Intermountain Health Good Samaritan Hospital* (Lafayette, Colo.)
  • Intermountain Health Intermountain Medical Center (Murray, Utah)
  • Intermountain Health LDS Hospital* (Salt Lake City)
  • Intermountain Health Lutheran Hospital (Wheat Ridge, Colo.)
  • Intermountain Health McKay‑Dee Hospital* (Ogden, Utah)
  • Intermountain Health Park City Hospital* (Utah)
  • Intermountain Health Riverton Hospital (Utah)
  • Intermountain Health Spanish Fork Hospital* (Utah)
  • Intermountain Health St. George Regional Hospital* (Utah)
  • Intermountain Health St. James Hospital (Butte, Mont.)
  • Intermountain Health St. Vincent Regional Hospital* (Billings, Mont.)
  • Intermountain Health Utah Valley Hospital* (Provo, Utah)

“These results reflect both commitment and compassion across Intermountain,” said Cara Camiolo, MD, chief quality and safety officer for Intermountain Health. “Earning an ‘A’ helps validate our caregivers’ efforts toward disciplined teamwork, transparency, and speaking up for safety every day. Whether a hospital achieved a Straight ‘A’ or made important progress this spring, each result represents caregivers doing the right things, for the right reasons, to create an ‘Always Safe’ culture.”

The Leapfrog Hospital Safety Grade evaluates hospitals on performance related to infections, medical errors, accidents, injuries, and the systems in place to prevent harm before it occurs. Hospitals receive a letter grade from A through F based on up to 22 national safety measures drawn from CMS data, the NHSN, and the Leapfrog Hospital Survey.
